Out of the box the game is letter-boxed to 16:9 by default, per PCGamingWiki, so ultrawide players see black bars full-time until they either edit user.ini to enable custom aspect ratios or use Flawless Widescreen, whose "South Park: The Stick of Truth" module was confirmed present in the current public repo listing. PCGamingWiki rates true ultrawide support as only "hackable," and does not confirm the FOV opens up correctly rather than just cropping/zooming, so this is rated conservatively as fix-required/poor rather than good.

user.ini aspect ratio unlockManually set support_aspect_ratio_custom=true and matching resolution values in user.ini; letterboxed to 16:9 without this edit, and even with it the game is not confirmed to render true hor+ ultrawide.

Confirmed via PCGamingWiki that the game letterboxes to 16:9 by default and lists ultrawidescreen support as 'hackable' via Flawless Widescreen; confirmed a matching FWS module is live in the current repo XML. Could not confirm from primary sources whether the FWS fix produces true hor+ scaling versus a cropped/zoomed view, so FOV behavior and overall rating were kept conservative (narrow/poor) pending further evidence.
